Get the care you need with our comprehensive in-house membership plan. The Loyalty Advantage Plan is a discount program not a dental insurance plan and is available for use only at Ridge Dental + Aesthetics.

Coverage includes:

Diagnostic and X-rays

The following diagnostic services are covered 100% in the plan:

• Comprehensive Exam
• 1 Annual Exam
• 1 Emergency Exam
• 4 Bitewing X-rays
• 1 Periapical X-ray
• Complete Series of X-rays or Panorex

Preventative

The following preventative services are covered 100% in the plan:

• Child Prophy (2 Cleanings/yr)
• Adult Prophy (2 Cleanings/yr)
• Fluoride (2 per year, no age limit)
• Oral Cancer Screenings

The following preventative services are discounted 20% off in the plan:

• Additional cleanings per year
• Dental Sealants

* Dual plan is for 2 adults (immediate family members 13 and up)

** Family Plan includes family members and children under 18 or children enrolled in college full-time until the age of 23.

Program Guidelines

• Patient's portion of bill is due at time of service
• Cannot be used in conjunction with any dental insurance plan
• Care credit cannot be used to pay premiums or for treatment
• No refund of premiums will be issued at any time if participant decides to not utilize dental loyalty plan

Programs Exclusions and Limitations

This program is a discount plan and not a dental insurance plan. It cannot be used for:

• Referrals to specialists
• Treatment which, in the sole opinion of our doctors, lies outside the realm of their capability.
• Hospitalization or hospital charges of any kind
• Cost of dental care which are covered under automobile or medical insurance
• Services of injuries covered under worker's compensation
• In conjunction with any other dental or discount plan

Loyalty Plan premiums must be paid with cash, check or credit card.
